CoTE petition

Use this form to request to take a course for CR/NC instead of a letter grade. NOTE, CR/NC courses will only count as electives and may not be used to meet degree requirements.

CR/NC Request form

Use this form to request to change majors within the College of Education or to request updates to your concentration areas.

EDUC Program Change form

Use this form to request an extension to complete coursework.

Incomplete grade contract

Use this form to request to add a course after the deadline (but within the same semester the course is being taken), request a section change or request a change to the number of credit hours for variable credit hours courses.

Late Course Change Form

Use this form to request to drop from a course after the deadline.

Late course drop form

Use this form to request to take more than 18 credit hours in a fall or spring semester or more than 9 hours in a summer semester.

Overload request

Use this form to request to return to a College of Education major IF you are:

• Any University of Illinois UNDERGRADUATE student who voluntarily “stopped out” for more than two calendar years AND has not exceeded the maximum terms of enrollment eligibility.
• Any University of Illinois UNDERGRADUATE student who voluntarily “stopped out” for one semester or more AND left while not in good academic standing.
• Any University of Illinois UNDERGRADUATE student on drop status or required to spend time away from campus due to inadequate academic progress.

Re-Entry Petition

Use this form to submit your proof of working at least 50 hours with persons with disabilities

Special Education Hours Verification form

Use this form to request re-enrollment in a College of Education major IF:

• You voluntarily withdrew or “stopped out” for a period of at least two semesters (not including summer) but not more than two calendar years.
• You left in good academic standing.
• You have not exceeded the maximum terms of eligibility (10 semesters for students who enter UIUC as freshmen, 6 semesters for students who transfer to UIUC from external institutions).

Statement of Intent to Return Request

Use this form to indicate if you are taking a semester or more away from your College of Education program studies.

Stopping out form

Use this form to request one of the following:

• A course approval or substitution to a required course in your major.
• A gateway petition to allow you to move into the Professional Education Sequence without having all requirements completed.
• A request for CR/NC after the deadline has passed.
• A request for a grade replacement after the deadline has passed.
• Any other type of request for which there is no dedicated form or petition.

Undergraduate Student Petition

Use this form to request to take less than 12 credit hours in a semester.

Underload request

Use this form to declare up to two (2) campus approved minors to your program of study. NOTE- First year students are not able to declare minors.

Minor Declaration Form

Use this form to request a course substitution to your existing campus approved minor.

Minor Modification Form

Use this form to request to drop a campus approved minor from your program of study.

Minor Cancelation Form

Contact your advisor for this form if you are considering withdrawing from or canceling your registration from the University of Illinois.
