Early Field Experiences

Evaluation Form

CoTE Early Field Experience Evaluation {EXAMPLE ONLY, Actual Form on Portal}

CI 405 (Year 1 Fall)

By the end of the semester, all students will complete the following: Submit hours online in student portal; Submit paperwork to room 120 (CoTE Bloodborne Pathogens Questionnaire, two lesson evaluations, with student name and all required signatures); Ensure cooperating teacher has completed the online final evaluation through the cooperating personnel portal.

Failure to turn in paperwork and complete online time card will result in a missing grade for CI 405 and an inability to advance to the spring placement.

EDPR 250EL (Year 1 Spring)

By the end of the semester, all students will complete the following: Submit hours online in student portal; Submit paperwork to room 120 [CoTE Bloodborne Pathogens Questionnaire (one per team) and three lesson evaluations, with student name and all required signatures]; Ensure cooperating teacher has completed the online final evaluation through the cooperating personnel portal.

Failure to turn in paperwork and complete online time card will result in an inability to advance to the fall placement.
