Application to Major

Information pertaining to Inter-College Transfer to Education Major as well as the application.

Application to Minor

Information pertaining to Education Minor admission as well as the application.

Area of Concentration Change Request

This form is used by Elementary Education  and Early Childhood Education students who wish to change their Area of Concentration.

Late Course Add/Section Change/Credit Change

This form is used by all College of Education undergrads who wish to add a course or change a course credit or section. An undergrad petition is not required.

Late Course Drop

Only for documented and extenuating circumstances past the deadline. An undergrad petition is not required.

Credit/No Credit

To request by the campus deadline; if past deadline must be accompanied by undergraduate petition.

Underload Request

This form is used by all College of Education students who wish to take less than the 12 hour minimum requirement for the fall or spring term.

Statement of Intent to Return

For students who stopped out for 2 semesters  but not more than 2 years, left in good standing, and not exceeded max term of eligibility.

Incomplete Grade Contract

For students requesting an Incomplete- this form will adhere to the campus academic deadline of “F by rule”. The student is responsible for obtaining instructor signature before submitting to the SAAO office.

Withdrawal/Cancellation Form

These are campus forms. Students should be aware of campus deadlines for submitting these forms.

  • Cancellation form is for students who never attended class or utilized campus services.
  • Withdrawal form is for students who attended class or utilized campus services

Parkland Concurrent Enrollment

Students requiring financial aid, will need this additional form.

Adviser signature is required on all Parkland Concurrent Enrollment forms.

Grade Replacement

This is a campus form. Requests should be submitted to the College of Education by the campus deadline.  If submitted past the campus deadline, the request must be accompanied by undergraduate petition.

ISBE 73-60

This is an Illinois State Board of Education form. Students should review ACT/SAT in lieu of TAP for more information including how to complete the form and who it should be submitted to.

Stopping Out

This form is for students who are leaving for one semester or more (excluding summer terms).
